ABSTRACT

Female 34 year old patient with insidious upper abdominal pain and progressive dyspnea that becomes more evident after Valsalva maneuver and lying down. Seeks medical attention and specialist orders chest x-rays that show intestinal handles in right hemi thorax. She has prior history of car accident 13 years ago. Incidence of herniation of the right diaphragm secondary to trauma is low due to the protection given by the liver to the right hemi diaphragm, which is why an important suspicion is needed in order to establish diagnosis. A chest x ray is the first step in the evaluation of the patient, in spite its low sensitivity, especially in right hemi diaphragm hernias.
