Radiographic and ecographic correlation in acute appendicitis

ABSTRACT

Objective: To establish reliability of radiological studies of abdominal flat plate X-ray and pelvic ultrasound in patients with acute appendicitis and its complications.
Patients and method: We performed a descriptive study and data analysis, which included all patients admitted and surgically treated with a diagnosis of acute appendicitis at the Regional Hospital General Ignacio Zaragoza, ISSSTE, in a period of six months from November 2009 to April 2010. We included male and female patients of all age groups. All patients had probable diagnosis of acute appendicitis confirmed by pathologic report. Data as age, sex, radiological report, abdominal flat plate x-ray, ultrasound and pelvic pathology report were analyzed. It was established correlation between the radiological diagnosis by abdominal flat plate X-ray and pelvic ultrasound. Pathology report was used as the standard to determine the reliability of reporting.
Results: One hundred eighty-three patients were accepted into the study protocol; all underwent appendectomy and all cases showed inflammatory changes that confirmed the diagnosis in the surgical specimen. In the pathology report all patients with acute appendicitis changes in any of their evolutionary rates were accepted. It was shown that the certainty of pelvic ultrasound in the radiology department of the Regional Hospital General Ignacio Zaragoza, ISSSTE, was of 81% and that of abdominal flat plate X-ray was of 39%. Plain, abdominal radiography and ultrasound showed higher frequency of signs, such as perforation or collections (93.7% for ultrasound and 81.2% for abdominal flat plate X-ray).
Conclusions: Ultrasound is useful in the early diagnosis of acute appendicitis and its complications. Abdominal flat plate X-ray is useful when the abdominal pain may suggest some other diagnosis or complications of appendicitis.
