ABSTRACT
This is the case of a male of 9 years with the diagnosis of hyperactivity, impulsivity and aggressiveness. At the age of six years all of these symptoms were increased and inappropriate language and motor tics as left head deviation in a stereotyped way were added. He saw a Psychiatrist who diagnosed TDAH and treatment with metilfenidate was started, with this scheme motor tics got worse, so treatment changed to fluoxetin because anxiety disorder were included to the diagnosis. He is referred to the department of Pediatrics Neurology of the Hospital Civil Fray Antonio Alcalde for poor control, where Gilles de la Tourette syndrome (GTS) was diagnosed, starting treatment with haloperidol.GTS is a chronic neuropsychiatric disorder that starts before the age of 18, characterized for multiple motor tics and at least one verbal tic. The motor tics more common affects the head presenting continuous flicker; while verbal tics are more common during adolescence and they tended to decrease at adult age. Its etiology and physiopathology are unknown, the theories postulate an alteration in dopamine, serotonin and norepinephrine. Its treatment consists in the use of dopamine antagonist. The prognosis depends of the severity and frequency of the tics, improving in adult age.
