Treatment of the open fractures of tibia by means of unreamed locking intramedullary nailing

ABSTRACT
We report the clinical and radiological results of immediate treatment of 20 open fractures of the tibia using unreamed locking intramedullary nailing, with a mean follow up of 21 months. Fourteen of the patients were male and the other 6 female, with a mean age of 43 years. The most frequent cause of fracture was traffic accidents. Of the 20 lesions studied, 6 were of grade I, 8 of grade II, 4 of grade IIIA and 2 of grade IIIB. There were no cases of grade IIIC. All patients were treated within 8 hour after the lesion. In all cases, fine (9-10 mm) Grosse-Kempf unreamed locking nails were used. All patients received antibiotherapy with cefuroxime and tobramicin for at least one week. All lesions of soft tissues were covered during the first week after the lesion had been produced. No cases of pseudoarthrosis or deep infection were observed. In three cases there was a shortening greater than 1 cm and one case consolidated with a valgus deformity of 8 degrees. In the light of the results and although our series is small, we conclude that immediate unreamed locking intramedullary nailing is an effective and safe method for treating open fractures of the tibia, especially those of grades I, II and IIIA.
