Leiomyosarcoma of the vulva. A case report with 22 years of follow up

ABSTRACT

We report a case of vulvar leiomyosarcoma of low grade, with 22 years of follow up, in a teenager of 14 years old. Initial treatment was an insufficient surgery for a mass of more than 5 cm. In a diameter and the evolution of the disease has been with four local recurrences more than 5 cm each one; two after surgery and two after surgery plus chemotherapy and surgery and surgery plus radiotherapy respectively. She is alive without evidence of disease after two years from her last combined treatment. We emphasized the rarity of these tumours and the fact that our patient, is the youngest with vulvar leiomyosarcoma reported in the bibliography consulted. Treatment of these cases is radical local excision followed of radiotherapy mainly, in neoplasm’s more than 5 cm in diameter; with high degree and with infiltrating margins. The value of postoperative adjuvant chemotherapy is uncertain. In accordance with the behaviour of the tumour in our patient, there is a high risk for a new recurrence.
