Influence of time of administration of the antidiabetic effect. Chronopharmacology of repaglinida

ABSTRACT
Background: The chronopharmacology studies the influence of timing of administration of drugs on their effects. The antidiabetic medications usually are prescribed at the same dose regardless of time of day and only adjusted according to the amount of food consumed and therapeutic efficacy. However we do not know the chronopharmacology of antidiabetic drugs.Objective: To determine the influence of time of administration on hypoglycaemic effect of repaglinide in volunteers.
Method: Comparative, randomized, double-blind, placebo-controlled trial, in which 1 mg of repaglinide was administered 5 min before breakfast, lunch and dinner. All meals were standardized in composition and caloric intake to 440 kcal. Capillary blood glucose was measured before and 2 hours of each meal with a validated glucometer.
Results: With placebo there was a significantly increase in blood glucose two hours after lunch and dinner, without any change after breakfast. Repaglinide significantly lowered glucose concentrations after two hours of the three meals, its effect was significantly greater in the breakfast and lunch than dinner. The magnitude of the effect of repaglinide was associated with blood glucose level prior to breakfast and lunch but not dinner.
Conclusions: Repaglinide has an hypoglycemic effect approximately doubled in the breakfast and lunch over dinner.
