ABSTRACT

Background: Alcohol- related disorders are highly prevalent. Their presence is associated with high levels of psychiatric comorbidity and early mortality. It has been reported a prevalence of delirium tremens of 5% in patients with alcohol withdrawal syndrome (AWS). The severity of AWS assessed with CIWA-Ar scale, has proved useful for evaluation of symptoms of AWS.
Objective: To evaluate the usefulness of CIWA-Ar scale as a method for diagnostic predictor of delirium tremens in patients with AWS.
Patients and methods: Study cohort, prospective, longitudinal. We studied 100 patients with AWS and divided in 2 groups according to severity given by CIWA-Ar scale and were hospitalized and evaluated for presence of delirium tremens during hospitalization.
Results: We included 51 patients with mild to moderate AWS and 49 with severe AWS. Patients with delirium tremens were presence of 36 (36%), 15 (29.41%) had mild-moderate scores CIWA-Ar, and 21 patients (42.86) had severe CIWA scales. 64 patients (64%) did not develop delirium tremens during hospitalization, 36% (70.59%) were patients with mild-moderate CIWA and 28 (57.14%) with CIWA severe AWS (P = 0.16).
Conclusions: The CIWA-Ar scale does not predict the development of delirium tremens, so it is not useful for diagnostic of delirium tremens in patients with AWS.
