Correction of caudal septal deviation with cross technique

ABSTRACT
Background: Correction of the caudal septal deviation is a great challenge; this defect can cause obstruction or aesthetical problems to the nasal base. Regardless the technique we use, it must be aimed to the reposition of the cartilage in the midline, to preserve the function, nasal tip support and the septo-collumnelar aspect.Objective: To describe the technique of the cross incision for the correction of caudal septal deviations. To evaluate the improvement of ventilatory function in patients with nasal obstruction secondary to septal deviation, with this technique.
Patients and methods: We performed a prospective quasi-experimental study of patients with diagnosis of caudal and convex septal deviation, previously documented with clinical examination and photographic record. The same surgeon performed the surgical procedure. Two different physicians evaluated patients two weeks, and one and three months after the procedure.
Results: This study included 15 patients with caudal septal deviation, with ages ranging from 16 to 37 years (mean 24.13 ± 10 years); 70% of patients reported improvement of nasal ventilation at two weeks, the rest referred improvement one month and three months after surgical procedure, with p = 0.001. Deviation was corrected in all patients, and they improved aesthetics of the nasal base.
Conclusions: The cross incision is a useful and simple method to correct the caudal septal deviations.
