ABSTRACT
Introduction: The HELLP syndrome complicates 0.2 to 0.6% of pregnancies, pregnancies with preeclampsia 4-14% and 50% pregnancies with preeclampsia. The most common symptom: headache. In the postpartum period presents the first 48 hours. Most have a history of preeclampsia. The laboratory diagnostic criteria are low levels of platelets, AST› 70 U/L, DHL> 600 U/L. The main hematologic complications are bleeding requiring blood transfusion from to CID.
Objective: Describe the evolution of a group of patients admitted to the Intensive Care Unit with a diagnosis of induced hypertension pregnancy complicated by HELLP syndrome during the postpartum period at the Hospital of Gynecology and Obstetrics Maternal-Child Institute of the State of Mexico.
Study design: Cases series, descriptive, retrospective, transversal and observational.
Results: We studied 21 patients, average age 24 ± 4.8 years; 43% with three or more pregnancies, 90.5% had no history of Ehyeh, 100% diagnosed with Ehyeh; 80.9% diagnosed in the puerperium. Headache in 57.1%. Complications occurred in 47.6%; 70% hematologic, 10% liver and kidney, respectively, and 20% cardio. There were no deaths.
Conclusions: The main risk factors were age between 22 to 26 years, have three or more pregnancies. No relationship was presented a history of prior Ehyeh pregnant patient had no history of diabetes mellitus. Severe preeclampsia, 76% of patients, which were complicated by HELLP syndrome. HELLP syndrome type one was made in 38.1%, type two, 52.4% and type three in 9.5% of all patients studied. The 57.1% had headache at the time of diagnosis, 52.4% epigastralgia, nausea and vomiting 23.8% and 4.8% visual disturbances. No patient had jaundice. The 47.3% had complications. The most frequent hematologic, anemia in 70%, 100% required blood transfusion. Acute renal failure was present in 10%. Requiring peritoneal dialysis 10%, and 20% hepatic cardiopulmonary.
