Spontaneous bilateral quadriceps tendon rupture in a healthy patient

ABSTRACT
Purpose: We report the case of spontaneous bilateral quadriceps tendon rupture in a healthy patient, its treatment, one year follow up and review of the pathophysiology.
Case report: The patient is a 65 years-old healthy male who practiced marathons and used to run an average of 25 miles per week. He denied a history of systemic diseases and his laboratory tests were normal. He referred intermittent bilateral knee pain for nearly 6 months treated with naproxen. The patient suffered a spontaneous bilateral quadriceps tendon rupture while running. He immediately went to the emergency room where the diagnosis was established by the physical exam and image studies. He underwent surgical bilateral quadriceps tendon repair using polyester multifilament Krackow suture technique with bony tunnel sutures in the patella. He had a good postoperative result with no complications. The patient came back to his normal sport activity by finishing a marathon twelve months after surgery.
Conclusions: Spontaneous bilateral quadriceps tendon ruptures are uncommon soft tissue injuries. They are highly correlated with systemic, endocrine, autoimmune and renal diseases, and in steroid abuse. They also have been reported in healthy individuals without predisposing factors. In this patient we could establish the diagnosis of spontaneous bilateral quadriceps tendon rupture after a degenerative tendinopathy. The predisposing factors identified were the patient’s age and the intense physical activity of the knees.
