Procalcitonin as a biomarker diagnostic of sepsis in adults

ABSTRACT
Objective: To determine the sensitivity and specificity and predictive values of Procalcitonin as diagnostic biomarker of Sepsis in older patients of Christus Muguerza Alta Especialidad Hospital in Monterrey, Nuevo Leon, Mexico. By this way I try to determine if this marker can help to differentiate between Sepsis and Systemic Inflammatory Response Syndrome (SRIS).
Methods: Were obtained registries of 135 patients, the totality of information about those patients was in the clinical records of Christus Muguerza Alta Especialidad Hospital during a period of January 2009 to August 2011. Each one must had the initial diagnostic of Fever of indeterminate cause. On the other hand, each record must had the complete registries of vital signs, white blood cells counts, serum Procalcitonin levels and cultures of different body places.
Results: With the last information, I can determine that of 135 patients 58% (79) had the diagnosis of SRIS and 42% (57) don´t had SRIS. And of 79 patients with SRIS, 62% (49) had Sepsis and 38% (30) don´t had Sepsis. The values to determine Serum Procalcitonin as a biomarker diagnostic of Sepsis, were Sensitivity of 41%, Specificity of 70%, Positive Predictive Value of 69% and Negative Predictive Value of 42% (With the cuttof of 2 ng/ml).
With those values, the serum Procalcitonin couldn´t help us to differentiate between this two entities.
