ABSTRACT
The techniques of medialization of the middle turbinate seek to avoid synechiaes in the middle meatus, and therefore the recurrence of nasosinusal disease. Iatrogenic adherences between the nasal septum and the lateral wall of the middle turbinate to maintain the space of the middle meatus in the postoperative period besides affecting nasal respiratory flow, can have adverse effects on smelling. The objective of this article is to demonstrate a new technique of medialization of the middle turbinate without the sequelae of iatrogenic synechiae from the turbinate to the nasal septum. Seventy-five patients who underwent minimal invasion endoscopic surgery of the paranasal sinuses (FESS). The medial portion of the middle turbinate was thinned with the debridator. In all the patients a septumplasty was performed in the same procedure to remove a portion of the perpendicular plate of the ethmoid that makes up the middle septum. A transfictive suture was placed from one side of the middle turbinate to the turbinate, holding the nasal septum. All the patients were reviewed with a follow-up endoscopy 12 weeks after surgery. The middle turbinate remained medialized, without formation of synechiae. Visual follow-up, cleansing of the area as well as ventilation and drainage of the sinonasal cavities are performed in the postoperatory period.REFERENCES
