Rupture of extensor tendons in fingers in rheumatoid arthritis

ABSTRACT
The tendons and all joints in the hand and wrist are affected in rheumatoid arthritis, compromising the daily activities of patients. The tendons most frequently affected are the extensor digiti minimi, and the extensor digitorum of the fifth ring and middle fingers. The extensor indicis is the less affected. We studied 70 patients with rheumatoid arthritis from 1971 to 1999 at the ABC Hospital; 13 patients had rupture of extensor tendons. In every case the patients were treated surgically with tenosynovectomy, synovectomy and resection of the distal end of the ulna. Eight cases were treated with latero-lateral suture of the proximal and distal ends of the extensor digiti minimi to the extensor digitorum of the ring and, 4 cases with suture of the proximal and distal ends of the extensor of the fifth and ring fingers to the extensor digitorum comunis of the middle finger and reinforced with the extensor indicis proprius, and one case with graft of the palmaris brevis tendon to the extensor digitorum of the middle, ring and fifth digits. All patients had good results.
