Effectiveness of preventive analgesia associated to regional peridural anesthesia

ABSTRACT
Surgical trauma induces nociceptive sensitization which it is translated into an amplification and an elongation of postoperative pain. There are different forms of blocking it as for example preventive analgesia and pharmacological synergy. The objective of this study is to prove that regional anesthesia like peridural with fentanyl and lidocaine, associated to the administration of ketorolac IV, and with preincisional application of bupivacaine is of greater benefit than general anesthesia with preincisional application of bupivacaine.
Materials and method: Prospective study of 20 patients divided into two groups: (I) Regional anesthesia as peridural with lidocaine and fentanyl associated to the administration of ketorolac IV and preincisional bupivacaine infiltration. (II) General anesthesia with bupivacaine infiltration before surgical incision. Evaluation of pain by means of analogical scale of pain and comparison between both groups with the statistic method of lineal regression or minimum squares, an r: 0.88 is considerate of significance.
Results: During the first 12 hours in group I average in the analogical scale of pain was of 0.43 while group II average was of 3.273 (r › 0.88); in the recovering room values were of 0 and 4.35 respectively, with and earlier use of analgesics and a greater quantity of analgesics in the second group.
Conclusions: Peridural anesthesia plus sinergical use of analgesics as a preemptive form is superior to general anesthesia with preincisional infiltration of bupivacaine.
