Solid-pseudopapillary tumors of the pancreas

ABSTRACT
The report of solid pseudopapillary pancreatic tumors, defined as potentially malignant or real malignant tumor has increased, due to their wide identification and characterization as anatomopathological entity. The tumor has a 2% of frequency considering all exocrine pancreatic neoplasms. They grow slowly with no exact correlation between the histological aspect and biological behaviour. We present herein two cases of female patients (24 and 15 year-old / Case 1 -and Case 2), with excellent evolution after the post-surgical diagnosis of solid-pseudopapillary pancreatic tumors, at the ABC Medical Center, in Mexico City. The two patients presented with abdominal pain; one of them (case 2) after a slight abdominal trauma. The tumors measured 14 cm and 12 cm respectively. Case 1 was predominantly cyst and case 2 was predominantly solid. The two cases had similar histological aspect: solid zones alternating with papillary areas and myxoid change. The immunostain was positive for vimentin and variable expression for neuroendocrine markers was found. The tumor cells expressed cytokeratin, and some of them were positive for progesteron receptors. The presence of ductuloinsular complexes in the residual pancreatic parenchyma was an interesting finding highlighted by the immunohistochemistry.
Conclusions: Solid-pseudopapillary tumors present mainly in young women. Their solid-cystic gross aspect, its easy surgical resection, the large size, and the solid-papillary histological aspect, suggest the diagnosis and avoid excessive surgical resection.
