Splenectomy in hematologic disease. Six years experience at the ABC Hospital

ABSTRACT
This is a retrospective, observational and longitudinal study of the patients with hematologic disease treated with splenectomy by several surgeons at the ABC hospital during a 6 year period. There were sixteen patients, 8 were diagnosed with idiopathic thrombocitopenic purpura (50%), 3 with non-Hodgkin lymphoma (18.8%), 2 with Evans’ syndrome (12.5%), 1 spherocytosis (6.2%), 1 with hairy-cell leukemia (6.2%), and 1 MALT lymphoma (6.2%). Thirteen elective and 5 emergency procedures were performed, 13 with open surgery (mean operative time 150 min) and 3 laparoscopic procedures (mean operative time 315 min). Accessory spleens were found in 18.75% of the cases. The mean hospitalization time was the same in both groups. In the laparoscopic group the morbidity and mortality rates where higher than in the open group. Six patients received the pneumococcus vaccine postoperatively. Surgery for hematologic disease has acceptable morbidity and mortality rates. Open procedures are indicated for patients who require emergency surgery.
