Model of early intervention in postpartum depression in a third level hospital

ABSTRACT
Background: 10 to 13% of new mothers are affected by postpartum depression. Interaction between depressed mother and her child is characterized by lack of responsibility, passivity or avoidance. Early psychological interventions are beneficial since alterations in mother-infant interaction affect decisively behavior and development of the child.
Objectives: To establish a multidisciplinary early intervention model in a tertiary care hospital for mothers affected by postpartum depression to reduce morbidity and improve the quality of mother-infant relationship.
Patients and methods: We performed a study of 17 patients with symptoms of postpartum depression hospitalized for two weeks in Maternal-Fetal Medicine Unit, CMN 20 de Noviembre, ISSSTE, in a room shared with their infant. Mother and son had a multidisciplinary care during hospitalization. Postpartum depression was determined using the test of Edinburgh from postnatal depression. The quality of the mother-child relationship was studied through the scale of mother-infant interaction (Bethlem).
Results: When we assessed the initial relation between the data obtained in the Edinburg and Bethlem tests a positive correlation was observed between the depressed new mother and her difficult to establish a good relationship with her son (
p = 0.01). After the multidisciplinary treatment, on the scale Edinburgh we appreciated an average reduction of 11 points from the initial score. On the scale of Bethlem, the average reduction was 2.65 points.
Conclusions: Hospitalization in an area specially designed to achieve the relaxation of the mother, and a multidisciplinary and timely attention contribute significantly to decrease the symptoms of postpartum depression and improve mother-infant interaction.
