Causes and prevalence of pleural effusion in the Hospital General Naval de Alta Especialidad

ABSTRACT
Background: In Mexico the pleural effusion is very frequent with a prevalence of 400 cases per 100 000 inhabitants in the year; the most common causes are congestive heart failure, pneumonia and cancer. The cytologic examination of the pleural fluid, because of its simplicity, is the most widely used method to diagnose malignant pleural effusion (up to 60 %).
Objective: To determine the prevalence of pleural effusion as well as understand their more frequent causes; evaluate the usefulness of the pleural fluid cytology for the final diagnosis.
Material and method: Retrospective, descriptive and cross-sectional study in which 67 cases were reviewed, reports of cytology of pleural fluid of patients with pleural effusion in the period between January 2005 and December 2010, at the Hospital General Naval de Alta Especialidad.
Results: Results were as follows: period prevalence was 13.4 for every 1 000 income in 5 years. The most frequent cause of pleural effusion was neoplastic: 43.5% cardiogenic 9.5 %, paraneumonic 22%, chronic kidney failure 13% and other 12%. Sensitivity and specificity of the pleural fluid cytology in the diagnosis were 97.7 and 43 %, respectively. The accuracy of the test was 79.1% with 2.3% false positive and 56.5% false negative results.
Conclusion: The utility of the cytology, with sensitivity of 97.7% and a specificity of 43%, was similar to that reported in the international bibliography.
