Modified pancreatogastrostomy and presentation of the pylorus after pancreatoduodenectomy: a case report and review of the literature

ABSTRACT
At present there are modifications in pancreatoduodenectomy reconstruction with clear theoretical and practical advantages that have enhanced the morbi-mortality of this major surgery. In this paper, we show the technical and physiologic advantage of pylorus preservation and pancreatogastrostomy recon-struction in a case of pancreatoduodenectomy. Male of 38 years of age, patients with a 2-month icteric and loss of weight symptoms, arrived at the Hospital Juarez de Mexico, in Mexico City, where a Vater ampulla carcinomas is found. It was decided to carry out a pancreatoduodenectomy. This trea-tment was complicated with pylorus preservation and pancreatogastrostomy. The final diagnosis was ampulla adenocarcinoma, stage III, (T1, N1, Mx). The patient had a satisfactory evolution, leaving the hospital at day 14. He has been treated concomitantly with chemotherapy and radiotherapy.
We suggest in this paper the use of pancreatogastrostomy reconstruction and pylorus preservation, because the first has the fewest indexes of anastomotic leaks, and the second reduces the possibility of postgastrectomy syndrome. These factors represent low morbidity and mortality in patients requiring a pancreatoduodenectomy.
