ABSTRACT
Among the disease of coronary circulation, there exist the anatomic anomalies, which have low incidence (3:1000). They have ± clinical repercussion concerning type.
This work is about a patient with congenital pulmonary disease who had angina pectoris. The diagnostic studies showed alterations in the left coronary ostium. Atherosclerosis of the same vascular system, and pulmonary, tricuspid valve diseases.
We carried out De Vega no the procedure for tricuspid for tricuspid disease and pulmonary in plantation for a mechanical prosthetic valve. It was necesary to make an enlargement on the pulmonary annulus with a synthetic flap of polytetrafluoroetilene (PTFE) and myocardial revascularization.
We used an arterial duct (left internal thoracic artery to the anterior descendent artery).
The patient had low cardiac output in the immediate post-operative period, and right pneumony. At present 4 months after surgery, he is asymptomatic (NYHA II).
