Peripheral blood frotis and organic alteration in preeclampsia-eclampsia

ABSTRACT
Introduction: The spectrum of preeclampsia-eclampsia has a common physiopathological substract, that is lesion of the endothelium. The consequence of this damage develops a thrombotic-hemolytic microangiopathy, encouraging specific organ alteration. Peripheral blood frotis its a simple study to support our diagnosis and the prognosis of this entity.
Objective: The establishment of a clinic-pathologic relation ship among patients with preeclampsia-eclampsia. Relation morphological changes in the blood frotis and organic alteration.
Material and patients: We designed a prospective, transversal study comparing the patients in the Intensive Care Unit (ICU) with preeclampsia-eclampsia between June 1999 and February 2000. This patients were treated according to the hospital’s protocol including blood frotis when they arrived. Our method consisted of the review of files, dividing our population in two groups: Group A: patients with negative frotis, and a Group B: patients with positive frotis. We calculate and compared both groups using Student t test.
Results: We included 77 patients, nine (12%) with negative frotis identifying a Group A organic alteration and sixty-eight patients (88%) with positive frotis documented 56 in Group organic alteration. Odds ratio (OR) is 34, with significant differences in hematological and renal variables. We calculated that the possibility of a patient with preeclampsia-eclampsia is 34 times higher associated when with positive frotis. For this reason, we propose this test as routive for diagnosis and prognosis for preeclampsia-eclampsia with a simple, affordables test.
