Clinico-pathologic and therapeutic analysis in thirty cases of subungeal exostosis

ABSTRACT
Subungueal exostosis is present as a bony growth from the dorsal surface of the terminal phalanx and is encountered with a frequency of 0.003% in dermatological consultation. The 30 patients of the present series were from the consultation files of the Department of Dermatology at the Military Hospital, Mexico City, as well as private consultation. The patients ranged in age from 8 to 60 years, with an average age of 18.3 years. A total 56.6% of patients were in the second decade of life. There were 17 female patients and 13 male patients. Two of the cases presented in a finger and 23 cases (76.6%) were in the large toe; one was in another toe. Only one case had a recurrence of the lesion after surgical excision. The radiologic and histologic pictures were consistent with the diagnosis. The treatment of choice was local excision of the tumor through a transverse incision at the distal end of the toe tip immediately below the distal end of the nail. Dissection of exostosis was by osteotomy at its base, with preservation of the nail plate and nail bed.
