Embolization of right common iliac artery by firearm projectile with entrance in the chest. Case report

ABSTRACT
Embolization of the common iliac arteries is a rare complication in patients injured by firearm projectiles with entry wound at the thoracic level. We present the case of a male patient age 29 years wounded by a firearm projectile who simultaneously presented other injuries: entry wound at the level of the fourth left intercostal space, midaxillary line, without an exit wound. The subject subsequently presented signs of ischemia in pelvic members. Minutes later, the symptoms focalized toward the right pelvic member; a transoperative angiography was performed, finding the head of the projectile fully embolizing the right common iliac artery. The subject underwent successful resection of intravascular foreign body; unfortunately, he died one day later due to other complications. Suspicion of migration of firearm projectile, the possibility of embolization, performance of emergency arteriography with localization and subsequent extraction of the embolus are mandatory maneuvers, to be performed with a certain sen of urgency, which can save the affected extremity and even the patient’s life.REFERENCES
