Meaning of intraabdominal pressure in critically ill surgical patients

ABSTRACT
The objective was to study the relationship of elevated intraabdominal pressures and mortality in critically ill surgical patients. Measurement of intraabdominal pressures (IAP) was made in 92 randomized patients of patients 176 admitted to our Intensive Care Unit over 2 years. IAP was transduced via bladder pressures. The mean APACHE II score was 20 ± 4.52 at admission. Patients with postoperative hemoperitoneum, abdominal package, intraabdominal abscess, and postresuscitation visceral edema showed the highest IAPs. We demonstrates a lineal correlation between IAP and the number of deaths.
Pearson’s correlation coefficient was + 0.96. Intraabdominal complications were present in 89.6% of patients who died. The main causes of death were multiple organ dysfunction syndrome (MODS) and distributive shock. We conclude that IAP should be measured in critically ill surgical patients. This technique improves the diagnosis of abdominal compartment syndrome and intraabdominal complications and allows selection of patients for re-exploration.
