Surgical treatment of lagophthalmos by interposing an ear cartilage graft to elongate the levator palpebrae muscle

ABSTRACT
Eye exposure, conjunctival irritation and corneal keratitis are sequels of lagophthalmos secondary to facial palsy or an exophthalmos. Many techniques have been described to correct this condition. A new method to reduce the palpebral opening is presented here. A graft of ear cartilage is placed between the levator muscle of the eyelid and the tarsal plate. Oculi muscle were treated with this technique. 18 patients with paralysis of the orbicularis oculi muscle corroborated by electromyography and 8 patients with lagoftalmos secondary to exophthalmos. The palpebral opening decreased 2 to 3 mm in all cases. 23 patients showed a significant improvement of the ocular symptoms. 3 patients whose palpebral fissure could not be closed long enough to protect their eyeballs required a further elongation of the graft of the ear cartilage of 4 mm and a lateral tarsorrhaphy.
