ABSTRACT
We report the case of a patient, female, 11 months of age, from Tuxtla Gutiérrez, Chiapas, Mexico with an initial of clinical picture obstructive jaundice, acolia, coluria, and hepatomegalia with abdominal pain, increasing jaundice and presence of abdominal epigastric mass. After several laboratory tests and radiology studies (ultrasound, esophagogastroduodenal series and gammagraphy) the diagnosis was giant choledochal cyst type I in the Alonzo-Lej and Todani classification. This pathology must be treated by means of resection of the mucosal cyst using an internal approach and reconstruction with hepatoyeyunoanastomosis in Y of Roux. There were no complications and the patient is evolving. Ethiology, clinical behavior and diagnosis in children with this pathology is analyzed in the world literature.
