Cardiovascular disorders in a pediatric population and their relation to metabolic and anthropometric disorders

ABSTRACT
Background: Hypertension, ventricular hypertrophy and changes in the thickness of the intima-media of the carotid arteries can be identified in patients overweight or obese.
Objective: To report the increase of left ventricular mass, hypertension and intima-media thickness of carotid artery, metabolic syndrome and body mass index in pediatric patients.
Patients and method: We made a transversal, prospective, observational and descriptive study of pediatric patients aged between 6 and 18 years. All patients underwent medical history, anthropometry, determination of glucose, insulin, cholesterol and triglycerides, HOMA and QUICKI index calculation and left ventricular mass and intima-media thickness measurement by echocardiography.
Results: We studied 55 pediatric patients, of whom 28 (50.9%) were men and 27 (49.1%) women; 23 were children and 32 adolescents. With regard to BMI, 27 (49%) were between 25 and 29.9 and 14 (25.4%) were greater than 30. In the obese waist circumference was ›0.85, index of waist/hip ratio ›0.88, the HOMA-IR = 4.5 and QUICKI = 0.32. The left ventricular mass was 48.2 g and intima-media thickness was 0.42. Insulin resistance was more common in scholars and hypertension in adolescents. The left ventricular mass was found in children with hypertrophied insulin resistance, especially in males.
Conclusions: Children who are overweight and obese have left ventricular hypertrophy and incipient changes in the thickness of the intima-media of the carotid artery. Increased ventricular mass index is associated to insulin resistance.
