ABSTRACT
Breast cancer has increased exponentially in Mexico as in the rest of the world. Women who have had mastectomy require a safe reconstructive procedure with very good results. Development of breast reconstruction with autologous tissues has evolved over a 40-year period in which TRAM flap has prevailed as the most common method used worldwide, despite more sophisticated varying microsurgical techniques. There are only a few unrestricted contraindications for a TRAM, and usually abdominal tissue conditions are good enough to use it safely. This technique demands profound anatomical and physiological knowledge, mastery and expertise of the abdominal wall. Immediate breast reconstruction is very frequent today and it is possible to carry out a high-quality mastectomy and breast reconstruction in one surgery. Complications should be few considering the surgical procedure’s magnitude. Breast reconstruction with TRAM flap has passed the test of time as a standard procedure for autologous reconstructive practice.
