Perilymphatic fistula in the posterior semicircular canal, due to prominent jugular bulb

ABSTRACT
Perilymphatic fistulas are abnormal communications secondary to otic capsule disruptions that have relationship with craneoencephalic trauma and ear surgery. We report the case of a 39-year-old woman who had a perilymphatic fistula with an atypical presentation. She presented constant vertiginous sensation and pulsatil tinnitus of twoyear evolution without previous trauma or ear surgery. In CT scan, we detected the bone defect in relationship with a prominent jugular fosse and bulb. An asymmetric cranial venous system was observed on IM R and fleboresonance. The patient underwent surgery by right retrosigmoidal approach with occlusion of lateral sinus, we accessed into the jugular fosse for defect occlusion. Multiple anatomic variants can be observed in jugular bulb, in this patient we found a right high bulb, it can be possible that its physiologic pulsation caused the bone disruption in posterior semicircular canal.
