ABSTRACT
The experience in surgical management of 50 consecutive cases of female patients with abdominal deformity corresponding to Matarasso´s type IV classification, all of them had abdominoplasty with modified Grazer’s approach and an 8 to 24 month follow-up is presented. Technical details, like planning of vertical results in scar lateral portions, preservation of the innominated fascia, and creation of a new umbilical scar are reviewed. Results were considered good when the objectives were complied in abdominal wall reinforcement, diminishing in abdominal circumference and improvement of body contour. Major complication was deep venous thrombosis in the leg (2%) of one patient, and the minor complication were hypertrophic scar in eight cases (16%), umbilical estenosis in two patients (4%) and serums in two cases (4%), with patients satisfaction. It was concluded that the proposed technique is a good choice in these patients management, with consistent and predictable results. The preservation of the innominated fascia contributes to decrease the serum incidence and a modification of Matarasso´s classification is proposed.
