Experimental comparison between tenodesis of the medial canthal tendon with adhesive and cartilage button vs wire

ABSTRACT
Facial fractures occur as a consequence of high-energy impacts. Resulting bone fragments may injure structures such as the medial canthal tendon. An early and effective repair of such injuries makes it possible to avoid sequelae such as telecanthus and scarring. Various techniques to repair the medial canthal tendon have been described, using wire, nylon, plates and screws. Fixation with adhesives like butyl-2-cyanoacrylate has been widely used, with promising results due to its adhesive properties and the minimal inflammatory response elicited. However, there are no published trials in world literature. An experiment was carried out on 30 New Zealand rabbits in order to determine the efficacy in the fixation of the medial canthal tendon. It was deinserted in both orbits, and the control side was fixed with wire, and the orbit under study was repaired with adhesive and a cartilage button, in order to compare both techniques. Results showed a greater tensile strength (p < 0.0001) and a shorter surgical time in the tendon fixed with the cartilage button and adhesive. Inflammatory response was similar with both techniques. Adhesive-fixed orbits showed bone union, whereas wire-fixed orbits did not. Fixation of the medial canthal tendon with a cartilage button and adhesive was found to be a safe, effective, and quick procedure.
