Primary hyperparathyroidism: Surgical approach with minimal invasion

ABSTRACT
Primary hyperparathyroidism (PHPT) is a disease that has been detected more frequently in recent decades due to the use of auto analyzers in routine blood tests. Typically patients are asymptomatic or have very nonspecific symptoms. Most cases of PHPT are due to an adenoma of any of the four parathyroid glands. Traditional surgery involved the search of all glands to identify the one with the adenoma causing the disease. With the advent of nuclear medicine studies to detect the site of the adenoma, ultrasound techniques and minimally invasive surgery, PHPT can be cured with safe procedures, less complications, short hospitalization and reduced costs. We present the experience with the management of 22 cases of PHPT operated in a recent period of 10 years analyzing the lessons learned in handling this pathology in the light of new diagnostic and therapeutic modalities.
