Acute suppurative thyroiditis due to a pyriform sinus fistula

ABSTRACT
Acute purulent thyroiditis due to fistulae of the pyriform sinus is a very rare condition in the adult. We present the case of a 23-year old woman, who a few weeks after an upper respiratory tract infection, presented with fever, as well as pain and swelling of her left lower neck, along with clinical and biochemical evidence of thyrotoxicosis. Ultrasound and CT examination of her neck revealed the presence of several abscesses in both thyroid lobes. She underwent needle aspiration of her left thryoid lobe, obtaining purulent material that was negative for papillary carcinoma and did not grow any microorganisms. A barium swallow demonstrated the presence of a pyriform sinus fistula as the cause of her thyroid infection.
