Diagnostic correlation among fine needle aspiration biopsy, transoperatory histopathological study and definitive histopathological study of breast tumors

ABSTRACT
Objective: To determine the correlation between biopsy diagnosed with fine needle aspiration (FNAB), intraoperative histopathological study (IH) and final histopathological study of breast tumors, in a second level hospital population.
Patients and method: This was a prospective, longitudinal, and correlational study in which 571 patients were included over a period of three years. Patients were caught in the outpatient or emergency ward, with lower than 2 cm palpable tumors. All patients underwent FNAB, intraoperative and postoperative histopathological study. The variables analyzed included age, results of FNAB, of intraoperative histopathological examination, and of definitive histopathological study and treatment.
Results: Of the 571 initial patients, 192 were excluded because they did not meet the inclusion criteria. The most common benign breast conditions diagnosed by histopathological study were: fibrocystic mastopathy (184 cases, 48.5%), fibroadenoma (78 cases, 20.6%). Among the malignancies found, the most frequent were: infiltrating ductal cancer (27 cases, 7.1%), intralobular cancer (18 cases, 4.7%). FNAB sensitivity compared with intraoperative histopathological examination was 93% with a specificity of 100%. In the comparison of FNAB with histopathological study, the sensitivity was 85% and specificity of 100%. Statistical significance was observed in diagnostic methods (p ‹ 0.05).
Conclusions: FNAB is a study to screen and sort out patients who do not need to be hospitalized nor to be included in study protocols for preoperative exams; unnecessary costs are avoided and allows a faster diagnosis and definitive treatment.
