Efficacy of postoperative epidural analgesia with fentanyl and bupivacanine compared to buprenorphine and bupivacaine in patients submitted to hip arthroplasty

ABSTRACT
Background: Postoperative epidural analgesia is defined as the administration of local anesthetics with or without adjuvant drugs during the postoperative recovery period and whose aim is to sustain the individual freedom level of pain.Objective: To evaluate the analgesic quality and duration of bupivacaine combined with epidural fentanyl compared with bupivacaine combined with buprenorphine by the same route in single application, in the first 6 hours postoperatively in patients undergoing hip arthroplasty. This is a prospective, clinical, controlled, randomized study in Mexican patients of the Adolfo López Mateos Hospital, ISSSTE, during the period from January 1, 2009 to December 31, 2009.
Patients and method: A clinical study, comparative, prospective, randomized controlled during the period from January 1 to December 31, 2009. A total of 60 patients over 50 years of age, surgery for hip arthroplasty, were randomly assigned to three groups. We used an analgesic dose of epidural bupivacaine 0.125% 0.15 mg/kg-1 average 10 mg, group A was combined buprenorphine 3 g/kg-1, group B fentanyl 1 µg/kg-1, group C no opioid combined. Epidural catheter was removed. We assessed pain intensity by visual analog scale before application of analgesia and 6 hours afterward.
Results: There was a decrease in the intensity of postoperative pain in patients in group A compared with group C. Patients in group A had successful analgesia compared with groups B and C.
Conclusions: The addition of bupivacaine epidural analgesia combined with buprenorphine in postoperative patients undergoing hip arthroplasty is a good handling due to its quality and durability.
