Actynomicosis in the adenotonsilar tissue. Clinical case

ABSTRACT
Actinomycosis is a chronic suppurative infection caused by bacteria of the genus
Actinomyces. It is part of the normal flora of mouth and in immunocompromised patients with poor oral hygiene can cause diseases as an opportunistic pathogen. Clinical presentation includes cervicofacial form in 50%, abdominal in 20%, and 15% thoracic. The cervicofacial form occurs commonly in the mandible, tongue and other structures, but is rare in adenoids. This paper describes the adenoidal actinomycosis and presents a case of female patient 50 years old with bilateral nasal obstruction, on nasopharyngeal endoscopy an adenoidal mass was detected; biopsy reported nonspecific lymphoid hyperplasia and adenoidectomy was decided. Histopathological findings showed follicular lymphoid hyperplasia, with extensive squamous metaplasia associated with actinomycosis. Treatment with penicillin and intravenous clindamycin was prescribed for 10 days, continuing with amoxicillin/clavulanate plus clindamycin PO for 6 months.
