Dumbbell-shaped trigeminal schwannoma in an adolescent; total surgical removal via one-stage frontal orbitozygomatic approach

ABSTRACT
The objective is to describe a rare case of a trigeminal schwannoma in a child and the surgical procedure performed for therapy.
Case report: a 13 year old boy presented right sided hypoacusia and hemifacial hypoesthesia, dysarthric speech and cerebellar symptoms. Imaging studies revealed a unilateral dumbbell-shaped tumor, extending into the middle and posterior fossae, centered over Meckel´s cave. One-stage surgery was performed by an orbitozygomatic approach. The tumor was first debulked in the middle fossa, then peeled from the wall of the cavernous sinus, followed by extirpation of the tumor from the posterior fossa. The tumor was fully removed. Histopatological examination revealed a schwannoma.
Conclusion: trigeminal schwannoma is a tumor that rarely occurs in childhood. Although several, often multistaged surgical strategies have been reported in the literature, this tumor was eradicated by a one-stage orbitozygomatic approach.
