Diaphyseal fractures management of the femur wit centromedular nail, blockade with external fixator

ABSTRACT
Handling of femoral diaphysiary fractures using a centromedular nail, blocked with external fixer. Twenty nine patients were surgically treated in the Hospital Central de la Cruz Roja Mexicana “Guillermo Barroso Corichi”, in Mexico City, giving a follow-up to 18, and eliminating 11 due to being translated to other hospitalary units, with diaphysiary fractures of the femur which were complex, infra or supra-ithmical, in a period of time from Jan. 1, 1992 to Dec. 31, 1994. A clinical and radiographic diagnose was made, for posterior classification according to actual AO criteria. Two cases of simple fractures, 7 of wedge fractures, and 4 complex fractures were found. Twelve patients presented closed fractures and six presented exposed fractures (G-III of Gustilo). Surgical treatment was performed through postero-external access placing first the centro-medular nail and afterwards threaded AO external fixers. In the final evolution of the surgical treatment, a 100% G-IV consolidation of the fractures was attained between the 12th and 54th weeks, with a 100% of hypotrophy of the cuadriceps, 8 cases (44.4%) presented limitation of all mobility archs of the hip, and 14 patients (77.8) of the knee. This type of handling is aleatory when there are no bolted nails.REFERENCES
