ABSTRACT

Objective: To inform about one case of diaphragmatic rupture of three days, treated in two separated hospitals. Setting: Hospital General de Veracruz de los Servicios de Salud de Veracruz (SSA). Design: Informative trial. Patient: This is the case of 20 years old, Mexican male who crashed his car against another at very high speed, suffering multiple traumatisms. Underwent emergency exploratory laparotomy in the nearest hospital, then he was transferred to our hospital, and while he was in emergency room, he was found to be on respiratory distress and evident abdominal distention. On chest auscultation, peristaltic sounds heard on left hemithorax. A chest X ray taken from first hospital was conclusive. A new film with barium swallow done in our hospital confirmed the diagnosis. The patient underwent exploratory laparotomy for the second time, finding the stomach on thoracic situation with signs of partial necrosis.
