ABSTRACT
Pilomyxoid astrocytoma is a rare variant of pilocytic astrocytoma. Presenting more frequently in infants, this neoplasm presents distinct clinical and histological features, and has a more aggressive behavior (grade II according to the WHO classification). We are reporting the case of a 2 year-old girl presenting with a growing mass inside the 3rd ventricle. The histology showed a proliferation of monomorphous, small piloid cells, radially arranged around vessels in a pattern of perivascular rosettes in a myxoid matrix. These characteristics are relevant for the histological diagnosis of this tumor. Differential diagnosis was established with pilocytic astrocytoma and ependymoma. It is important to recognize this entity in order to make a correct diagnosis, as well as an adequate treatment and follow-up of the patient.
