Diffuse large B-cell lymphoma, with extensive plasmacytic differentiation and numerous Mott cells. A report of a little frequent case and brief historical note

ABSTRACT
A 64 year-old man presented with generalized lymphadenopathy. An excisional biopsy of a right inguinal lymph node showed a diffuse large B-cell lymphoma. Admixed with the large lymphoma cells were numerous Mott cells with large hyperchromatic nuclei and abundant eosinophilic cytoplasm and densely packed with numerous well-demarcated round small eosinophilic globules that completely obscured the cytoplasm and covered parts of the nucleus. The proportion of these two cell populations varied throughout the section. The cytoplasmic globules in the Mott cells stained bright red with Masson trichrome, black/blue with phosphotungstic acid hematoxylin, blue/violet with Gram, light red with Russell-Movat pentachrome and were weakly positive for periodic acid-Schiff (PAS). The large lymphoid cells were strongly immunopositive for CD20, CD79a, and Pax-5 and negative for CD3 and CD5. Mott cells were negative for Pax-5 but revealed diffuse strong nuclear staining for MUM1, membranous staining for CD138, staining in a rim attern for IgG and kappa light chain restriction. Most of the Mott cells showed membranous staining for CD20 and CD79a. The percentage of Ki-67 positive cells was 40%. Epstein-Barr virus encoded RNA (EBER)
in situ hybridization was negative. A diagnosis of DLCBL with Mott cell differentiation was rendered. A brief history of Sir Frederick Walker Mott is presented.
