ABSTRACT
Congenital vertical talus is an uncommon foot deformity that is present at birth and results in a rigid flatfoot deformity that must be treated to avoid deformity, pain and disability. The exact etiology of vertical talus is unknown, it could be idiopathic or associated with other neuromuscular abnormalities or genetic syndromes. Its clinical diagnosis is established through the analysis of the foot deformity, confirmed with x-rays and occasionally by MRI. Treatment generally consists of passive lengthening of the contractured soft tissue and external reduction of astragalus, which is maintained by four to six serial casting. In some cases, it could be necessary a less invasive approach to release the talus-escaphoid capsule, maintaining the correction with a percutaneus wire. Treatment could be different According to the age when problem is detected.
