Autologous fat injection for the treatment of unilateral cordal paralysis

ABSTRACT
Objectives: To establish the etiology and symptoms of unilateral vocal cords paralysis and to evaluate the effectiveness of intracordal autologous fat injection through symptoms resolution.
Material and Method: Descriptive, cross-sectional, retrospective and observational study. The population included patients with unilateral vocal cords paralysis in the period between 2001 and 2007, patients with this diagnosis were 71, from which only 34 met inclusion criteria.
Results: A total of 71 clinical records (34 useful) were analyzed, age range of major presentation was from 50 to 70 years old in 20 cases, sex distribution was 68% in women and 32% in men, about vocal cord affection the left one was present in 71% of cases and 29% in the right one, etiologically 61% of cases were postsurgical, 21% idiopathic and 18% neoplastic. The symptoms presented were dysphonia in the 34 cases and aspiration in 31 cases, from this universe, intracordal fat injection was performed in 26 patients, 1 patient didn’t accept surgical intervention and other 7 patients improved after voice therapy. From those who underwent surgery, in 13 patients (50%) dysphonia remitted, in other 10 patients it was reduced, and 3 cases of dysphonia remained without changes. Related to aspiration, in 18 it disappeared, in 6 cases (23%) it was reduced, 2 cases (8%) remained without changes and in 5 cases a second surgery was performed.
Conclusions: Surgery, performed with intracordal autologous fat injection, may be considered effective in expert surgeons in the treatment of unilateral cordal paralysis.
