Proposal for an index for handicapping malocclusions with deterioration potential and its application

ABSTRACT
The need in public health to select the patients who deserves orthodontic care of the second and third level has lead many authors to developed indices to determine occlusal problems with no dental origin, nevertheless any of these measures in a reliable way the craniofacialdental anomalies which are very important for the institutional care because of its trascendence, magnitude and severity. For this work the most common craniofacialdental alterations were obtained out of patients that were treated at the Orthodontics Department, General Hospital «Dr. Manuel Gea González» in Mexico City in 1996 doing a format which was enriched with new variables giving them nominal and ordinal values, and the summatory of these values suggests the level of possibility and treatment need of orthodontics at a hospital level; this new format was named «Index of handicapping malocclusions with deterioring potential» and was applied to 80 patients that asked for treatment in our hospital. The less score was gotten for a totally edentulous patient with a TMJ disorder, while the highest was for a patient with multiple facial clefts, this means according to the classification and punctuation proposed in this work, that the patient with multiple facial clefts deserves institutional orthodontic care, but necessarily the edentulous one.
