Dynamic testing in endocrinology: Precocious puberty

ABSTRACT
Precocious puberty is defined as the onset of puberty before the age of 8 years in girls and 9 years in boys; however, it may be affected by factors such as race, low birth weight and obesity. Early physical changes in puberty are the result of sexual hormones being produced by the activation of the hypothalamic-pituitary-gonadal axis. The onset of puberty is marked by breast development in girls and testicular enlargement in boys. Precocious puberty is classified as central, also known as GnRH-dependent puberty, and the main objective of this review, or peripheral, also known as GnRH-independent puberty. The first step in evaluating a child with precocious puberty is to obtain a complete family and personal history, followed by a physical examination. Diagnosis of precocious puberty relies on imaging techniques and laboratory testing. The GnRH stimulation test is considered the gold standard to evaluate the activation of the hypothalamic-pituitary-gonadal axis; peak LH or FSH values and LH/FSH peak ratios can be used to assess the response to the stimulus.
