Pulley suture to the treatment of septal caudal deviations

ABSTRACT
Background: In patients with caudal septal deviation it is observed the caudal septal border in one nare pushing beside the nasal spine, its etiology could be traumatic or congenital. The correction of the caudal septal deviation is a great challenge; these defects can cause obstruction or aesthetical problems to the nasal base. No matter the technique we use, it must be aimed to the reposition of the cartilage in the midline, preserve the function, nasal tip support and the septo-collumnelar aspect.Objectives: To describe the technique and to evaluate the effectiveness of the pulley suture for the correction of the caudal septal deviations.
Patients and methods: The caudal septoplasty was practiced in 13 patients previously documented in a clinic and photographic way. The procedures were made by the same surgeon with the technique of the pulley suture and the evaluated at two weeks, one and six months after surgery by three different physicians.
Results: The caudal septal deviation was corrected in the 100% of the cases; all the patients refused nasal obstruction 6 months after the procedure.
Conclusions: The pulley suture is a useful and simple method to correct the caudal septal deviations.
