ABSTRACT
Introduction: Bronchial rupture is an infrequent but potentially serious complication in blunt thoracic trauma, with an associated mortality of 30 %, whereas when accompanied with severe vascular injuries it may reach 50 %.
Clinical case: An eight year old girl was run over by a moving motor vehicle. On the sixth week following the accident a left thoracotomy was realized, and a total section of the left main bronchus was found with total occlusion of the lumen by fibrosis. An end -to-end anastomosis was carried out, obtaining a total and immediate pulmonary reexpansion.
Discussion: The rare occurrence of a ruptured bronchus, in combination with the symptomatic and radiological similarity with a pulmonary contusion can lead the physician to a late diagnosis, principally in a paediatric age. A late repair should always be attempted, even though a functional recovery is unpredictable.
