Postnatal varicella infection in a new born baby. A case report

ABSTRACT
BACKGROUND. Postnatal varicella generally occurs after the 12th day of life and spreads by respiratory transmission or direct contact; with a mortality above 37%.OBJETIVE. To describe a case of postnatal varicella.
CASE DESCRIPTION. A male newborn infant was admitted at age 13 days to the Neonatal Intensive Care Unit, with transposition of the great arteries; one day later he underwent surgical correction Jatene-type. On the 11th post-operative day, he presented fever and the following day a maculo-papular exanthem with vesicular hemorrhagic rash. Hemorrhagic varicella was suspected and treatment with acyclovir was begun. The diagnosis was confirmed by PCR (polymerase chain reaction) for varicella-zoster virus (VZV). He developed purpura, jaundice, with digestive tract and respiratory system hemorrhages, pneumonia and died seven days later.
CONCLUSIONS. Postnatal varicella is unusual, the diagnostic test is PCR for VZV; serology test (IgM VVZ) in newborn infants is useless.
