Pulmonary geotrichosis in a patient with renal transplantation. A report of a case

ABSTRACT
It is presented the case of a 35 year old female patient with chronic renal failure since age 21, who received a cadaveric renal allograft six months before admission. She was being treated with prednisone, azathioprine, and cyclosporine, achieving upper normal levels of cyclosporine. Ten days before admission she developed fever, productive cough with purulent sputum and dyspnea. The chest X-ray showed a bilateral alveolar infiltrate at the bases and a right lower lung opacity. A bronchoalveolar lavage with culture was done reporting Geotrichum sp as the only pathogen agent. She was treated with amphotericine B 750 mg and then ketoconazole for three weeks, adjusting cyclosporine doses with a complete resolution. Six months later, the patient remained asymptomatic. This case is important because Geotrichum is a normal respiratory tract habitant; however, in patients with high risk factors it can be pathogenic and must be suspected in immucompromised patients with lung disease that have not responded to conventional treatment.REFERENCES
