New Family Physician’s Competencies for Hospital: Care Coordination and Home Care for Chronic Patients. A Brazilian Experience

ABSTRACT
Technical progress in Medicine provides broad possibilities for practicing, although this is accompanied by complexity. The Family Physician at present possesses a new and important role in coordinating care for in-patients and managing home visits with a health team. This paper features the Family Physician profile for assuming this new role. The Sociedade Brasileira de Medicina de Familia (The Brazilian Society of Family Medicine, SOBRAMFA) experiences in training these physicians in their practice settings are described. The physicians require communication skills, to provide continuity of care, to adapt themselves in a practical way to each situation, and they need to make decisions wisely. All of these are virtues that are required for being efficient in the hospital milieu. Home visits are increasing as well, and Family Physicians are necessary for managing health teams and coordinating care at patients’ homes. For this, the physicians need to incorporate the core values of Family Medicine. Logistics and systematic schedules for delivering care are needed for the new model of home visits. The new role of the Family Physicians brings satisfaction for patients and families and achieves results for hospitals and insurance companies, because the model affords the holistic approach for practicing Medicine, along with humanity, leadership in coordinating teams, and maintaining low costs.
